Rates & Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
What is my health insurance coverage and benefits?
Who administers the mental health coverage for my plan?
What is my deductible and has it been met?
Do I have a co-pay or co-insurance? If so, how much is it?
As a courtesy we file your claims with your insurance carrier. We have provided a list of insurance place each provider participates in; however, it is ultimately the patient's responsibiity to verify coverage with you insurance carrier. This list is subject to change.
Insurance plan inclusions/exclusions: Provider specific.
Sliding scale, waiving copays/co-insurance/deductibles, and discounts:
Per contractual obligation with insurance providers, we are contractually bligated to collect the patient portion of the payment due. Copays, co-insurance and deductible is collected upon check in at each appointment. Our office does not offer sliding scale services.
Cash, check and all major credit cards including Discover and American Express are accepted forms of payment.
If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you may be subject to cancellation fees.
